biological hazard
the trains on my toilets (reverse that) are absolutely horrible. so it came as no surprise to see this tape stuck over the door. biological hazard - danger of infection.
i suspect this isn't an official sign ...

It is indeed an official sign, and without wishing to dwell on the matter it means stuff didn't go where it is supposed to. Sadly an indication of the times we live in.
